Diagnostic Accuracy of Hyperdense Artery Sign in Early Detection of Middle Cerebral Artery Infarction: A Cross-Sectional Validation Study

Background Hyperdense middle cerebral artery sign (HMCAS) on a non-enhanced multidetector computed tomography (MDCT) scan is considered an important radiological marker in detecting acute arterial thrombotic occlusion, and it one of the earliest signs ischemic cerebrovascular accident (CVA). This finding has been observed within 90 min symptom onset. Modern approaches to patients with infarction emphasize early diagnosis management. Purpose To determine diagnostic accuracy hyperdense detection (MCA) non-contrast-enhanced MDCT using Magnetic Resonance Angiography (MRA) 24 h as gold standard for definitive diagnosis. Method A total 140 aged 35–70 years, referred radiology department Aga Khan University Hospital clinical suspicion infarction, were included. After patient underwent initial complete brain, which was performed Aquilion ONE 640 slice (Toshiba Medical Systems, Japan). Consultant radiologists, minimum 5 years experience brain imaging, interpreted images. Follow-up examination MRA confirm MCA infarction. Results Overall sensitivity, specificity, positive predictive value, negative HMCAS 96.20%, 93.44%, 95.0%, respectively. Conclusion study concluded that scans very high.
